24-h pH Measurement


 This clinical procedure is to measure the motility of the lower esophageal sphincter (LES) and the esophagus. Patients with suspected esophageal motility disorders and gastroesophageal reflux (GERD) may be scheduled for the procedure. The usual indications include difficulty in swallowing, unexplained chest pain or pre-operative testing (prior to antireflux surgery). The potential diagnoses include achalasia, diffuse esophageal spasm, nutcracker esophagus (or nonspecific motility disorder), and GERD.

The procedure takes about 1 hr and the patient should be fasting for 4-6 hrs before the study.

protocol is as follows:
1) Measurement of LES pressure and relaxation:
a) Placement of a solid-state catheter with 4 pressure sensors into the stomach nasally.
b) Assessment of the location, length and resting pressure of the LES using station pull-through method (all 4 sensors will be pulled through the LES).
c) Measurement of the LES relaxation during wet swallows: 5-10 wet swallows with the 2nd or 3rd pressure sensor located in the LES.
2) Measurement of the esophageal body motility
With the distal pressure sensor 5 cm above the LES, 5-10 wet swallows will be given to identify the contractile amplitude and pattern of the esophageal body.
3) Assessment of the upper esophageal sphincter pressure (UES)
a) Determination of the location and resting pressure of the UES using the pull-through method.
b) Assessment of UES relaxation with 3-5 dry swallows with the 2nd or 3rd sensor at the UES.

Report of major results:
LES resting pressure:
1) Percentage of LES relaxation during wet swallows
2) Distal esophageal contractile amplitude
3) Distal esophageal contraction pattern
